I am a fee-for-service/private pay or out-of-network provider for most Insurance plans. Services may be covered in full or in part by your health insurance or employee benefit plan.

Fees: 

Initial Session:  $225

60 Minute Therapy Session: $200



There is a sliding fee scale option for clients who are experiencing financial hardship and need/want weekly appointments.



Payment

All session fees are due at the end of each session.

Cash, check or any major credit card is accepted for payment.
Please make checks payable to: Useful Perspectives Therapy, or UPT.

Cancellation Policy

If you do not keep your scheduled appointment, and you do not give a 24-hour notification, you will be charged the full session rate for the missed appointment.

Insurance

I am an out-of-network provider for most plans. Services may be covered in full or in part by your health insurance or employee benefit plan. The client is responsible to pay me my full fee each session and is given a Superbill that is required by the Insurance companies to be submitted by the client for any reimbursement for that session.

Please check your coverage carefully by asking the following questions:
Do I have out-of-network, out patient mental health insurance benefits?
What is my deductible and has it been met?
How many sessions per year does my health insurance cover?
What is the coverage amount per therapy session?
